Named GP

All our patients are given a Named GP when they register with the Practice, if you are not sure who your named GP is then please ask at reception.

This does not mean that you can only be seen by your named GP, you can continue to see any member of the practice clinical team of your choice. We are aware that seeing one Doctor is not always possible or practical. All clinical staff within the practice can access your medical records so continued care can be administered.

 Summary Care Records

Patients who consented (by not opting out) to have a Summary Care Record created have now one in place which is updated regularly.

Please find below more information regarding Summary Care Records.

A Summary Care Record is an electronic record which contains information about the medicines you take, allergies you suffer from and any bad reactions to medicines you have had.

Having this information stored in one place makes it easier for healthcare staff to treat you in an emergency, or when your GP practice is closed.

Dr. Simon Eccles explains how Summary Care Records help in out-of-hours and emergency settings.

For more information please follow the link below:

If you wish to opt out of having a Summary Care Record please fill in the form below and hand into the surgery:

GP2GP record transfer

If you register or de-register from Dr Taylor and Partners your records will be transferred electronically with other surgeries who also have this facility in place.
